Myofascial Release
At Garner Pelvic Health in Greensboro, NC, we use myofascial release and advanced manual therapy to address restrictions in the connective tissue, or fascia, that can contribute to pelvic pain, orthopedic pain, nerve irritation, impaired mobility, and difficulty returning to normal activity.
Fascia forms a continuous three-dimensional network throughout the body, surrounding and connecting muscles, nerves, blood vessels, organs, and other structures. Because of this continuity, a restriction in one area can sometimes influence movement or symptoms somewhere else.
Our approach is not simply to treat the place that hurts. We evaluate how fascia, muscles, joints, nerves, breathing mechanics, scars, and the pelvic floor interact as part of the whole system.
Myofascial release is integrated into a comprehensive physical therapy plan and is not typically used as a stand-alone treatment.
What Is Myofascial Release?
Myofascial release is a form of hands-on manual therapy used to identify and treat areas of restricted mobility within the body’s soft tissues and fascial system.
Restrictions may develop following:
- Surgery
- Injury or trauma
- Pregnancy and childbirth
- Endometriosis or chronic inflammation
- Scar formation
- Repetitive movement or overuse
- Prolonged protective muscle tension
- Changes in posture or movement
- Orthopedic injury
- Periods of immobilization
Rather than applying the same technique to everyone, treatment is guided by your individual examination.
Our therapists assess how tissues move, where restrictions occur, and, most importantly, whether those restrictions are actually contributing to your symptoms.

Conditions That May Benefit From Myofascial Treatment
Pelvic Pain
Pelvic pain rarely involves the pelvic floor alone.
Restrictions involving the abdominal wall, hips, pelvis, diaphragm, low back, scars, or surrounding connective tissue can alter movement and increase strain throughout the lumbopelvic system.
Myofascial techniques may be incorporated into treatment for conditions such as:
- Persistent pelvic pain
- Pain with intercourse
- Pelvic floor muscle overactivity
- Vulvar or perineal pain
- Coccyx pain
- Sacroiliac joint pain
- Low-back and pelvic girdle pain
Endometriosis and Abdominal-Pelvic Restriction
People with endometriosis may experience pain involving far more than the reproductive organs.
Previous inflammation, protective muscle guarding, abdominal or pelvic surgery, and scar formation can all affect movement of the abdominal wall and surrounding soft tissues.
For patients with endometriosis, we may combine myofascial treatment with pelvic floor rehabilitation, scar mobilization, visceral techniques, breathing and pressure-management strategies, and orthopedic treatment as appropriate.
Manual therapy does not treat or remove endometriosis lesions, but it may play a role in addressing associated musculoskeletal and fascial impairments.
Surgical Scars and Adhesions
A scar is more than something visible on the skin.
Surgery can influence multiple layers of tissue and may alter the way the abdominal wall, pelvis, hip, or surrounding structures move.
We frequently evaluate scars following:
- Cesarean section
- Endometriosis excision
- Hysterectomy
- Abdominal or pelvic surgery
- Hip arthroscopy
- Orthopedic surgery
- Other laparoscopic or open procedures
Treatment may include scar mobilization, myofascial techniques, and exercises to restore normal movement between the tissue layers.
What to Expect
Each visit begins with a hands-on evaluation of how your tissues move, where restrictions occur, and how they relate to your breathing, posture, and pelvic floor. Gentle, sustained manual techniques are then applied to the areas that are actually contributing to your symptoms, followed by individualized exercises and strategies to help the changes last.
